Doctor Jay Park is a co-founder of Cosmetic Medical Training. He has been training medical providers on the botulinum toxin and dermal filler injection technique for over 10 years. Dr. Park is board certified in Emergency Medicine. He attended medical school at George Washington University in DC. He was the chief resident at his residency at Beth Israel Medical Center in NYC. We commonly receive phone calls and emails from physicians who want to break into the field of aesthetic injections. Many times, physicians just want to know what they need to do get started. Well, since we are the experts, here is your guide.
State Regulation
While all states allow licensed physicians (MD and DO) to inject Botox and fillers, the law vary greatly when it comes to the supervisory relationship between the physician and the mid-level provider (PA, NP or RN). There are other states to state variations about who can own a medical spa and how the business is structured. We will not get into the details of each state, but to recommend that you contact your state licensing board to ask for clarification as laws are constantly changing.
Market research
“How much should I charge for Botox?” is another common question. We always recommend the same: market research. Hire a friend or family member to walk into all the medical spas and medical offices that offer Botox and ask them about their price list and specials. You will then immediately know the range of pricing in your area. Use this guide to price yourself accordingly. Market research will also reveal where you might open if you do not currently have an office space.
Office space
We usually have two types of physicians: ones who currently have a physical office space and ones who do not (specialties include EM, anesthesiology, hospitalist, etc). Obviously, those with a pre-existing office space have an advantage as they have space and a patient list that they can market to. Those physicians who do not have an office space, we recommend the following: get a partner.
Your ideal partner is another physician with an office space. Ask to use her/his space during the weekend while you are starting out your business. Other partnerships can include a day spa or a hair salon with a private space. You and your new partner can both inject cash into your new startup and split the profits or you can simply rent space from your new partner. While the location is key, if your new partner has a huge client list, then that may be worth more than a high traffic location.
A few physicians have asked to conduct their training at home, while the location is being established. While your state licensing board may not dictate where you can practice Botox injection, Allergan, the maker of Botox, cares as they do NOT ship to a residential address.

Your end time depends on the number of models and your discussion with your trainer (typically 4 – 5 hours total).
Please note:
- The areas that you will inject will vary depending on the models that are available on your training day. If you have a specific area that you want to target, then we recommend that you assist us in finding patients who will request to have the practice area injected on your training day.
- Dysport®, Jeuveau®, Restylane®, of Juvederm® injections may also be taught at your training depending on your product purchase.
- We ask that you reach out to your office staff, friends and family to volunteer to become injection models. Our experience has been that you will have NO problem finding volunteers if you or your staff will announce on Social Media (Facebook, Twitter, etc) that you need injection models. Most of our trainees will ask the models to pay for the cost of Botox® and Dermal Fillers used on that training day.
- The cost of Botox® and filler is NOT included in the training price, because we won’t know how many models you will have at the training date. We typically recommend that you buy at least 2 vials of Botox® and 4 filler syringes.
- Your non-medical office staff should be a part of the training experience for them to gain valuable sales knowledge.

Frequently Asked Questions
Can physicians legally administer Botox without prior aesthetic residency training?
Yes. Any licensed MD or DO holding an active state medical license can administer Botulinum Toxins and Dermal Fillers. Medical boards require proper clinical training and documentation of competency, which our course certificate provides.
How are live models arranged for the In-Office Onsite Training?
We guide your practice manager on recruiting 4 to 6 patients, staff members, or friends/family for your training day. You perform injections directly on these models under 1-on-1 supervision from our clinical trainer in your office operatory.
What happens if I do not feel 100% confident after my 1-day course?
We offer an absolute 100% Retraining Guarantee. If you do not feel fully confident injecting basic treatment zones independently, we will provide additional follow-up tele-training sessions with our Medical Director at zero extra charge.
