What every provider should understand

• Diagnosis first. Therapeutic treatment starts with a clinical evaluation and a confirmed indication.
• On-label and off-label use. Botox is FDA-approved for specific therapeutic indications. Some uses taught in this course, such as TMJ and bruxism, are generally considered off-label. Off-label use needs clinical justification, informed consent, and your own legal responsibility.
• Units are not interchangeable. The dose units of Botox cannot be compared with, or converted to, the units of other botulinum toxin products.
• Know the product label. Read the current prescribing information and Medication Guide before you treat.

Boxed warning: distant spread of toxin effect

Botox carries a boxed warning. The effects of Botox and all botulinum toxin products can spread from the injection area and cause symptoms of botulinum toxin effect, including swallowing and breathing difficulties that can be serious. Symptoms have been reported hours to weeks after injection. Patients should be told to seek immediate medical care if they develop swallowing, speech, or breathing problems. 

Contraindications (from the prescribing information)

• Known hypersensitivity to any botulinum toxin product or any component of the formulation. Serious hypersensitivity reactions have been reported.
• Infection at the proposed injection site.
• For bladder injection: urinary tract infection, or urinary retention or elevated post-void residual in patients who are not routinely performing clean intermittent self-catheterization.
